This formula contains berberine, a pharmacologically active alkaloid that inhibits CYP3A4, CYP2D6 and CYP2C9 liver enzymes along with P-glycoprotein, and can raise blood levels of other medicines. Documented concerns include diabetes medicines (risk of hypoglycaemia), warfarin and other anticoagulants, cyclosporine, statins, and blood pressure and heart medicines including digoxin. The National Center for Complementary and Integrative Health reports that berberine exposure has been linked to a harmful buildup of bilirubin in infants and is likely unsafe for infants and possibly unsafe in pregnancy and breastfeeding — see NCCIH on berberine. NCCIH reports that evidence for berberine and weight is not conclusive, with effects seen mainly above 1 gram per day over more than 8 weeks in studies carrying a high risk of bias.
